The Speech Therapy Mod brings realistic speech therapy sessions, homework, milestones, and graduation celebrations to The Sims 4 families.

If you’re looking for more meaningful family storytelling, the Speech Therapy Mod by JellyPaws is a beautiful new addition to The Sims 4. This brand-new gameplay mod introduces realistic speech therapy visits for toddlers and children, giving young Sims a chance to build confidence, improve communication, and celebrate every milestone along the way.
Instead of treating speech development as a simple skill gain, the mod turns it into a complete journey filled with professional therapy sessions, homework, emotional moments, progress tracking, and even a graduation ceremony. It is a thoughtful feature for players who enjoy family gameplay with a little extra depth.
Speech Therapy Sessions Feel Like Real Appointments
Once installed, players can click on any toddler or child and choose Call Speech Pathologist (§75). A dedicated speech pathologist will travel to the household specifically for that child, greet the family professionally, and begin a private one-on-one therapy session.
Unlike many NPC visits, the therapist stays focused on their work instead of wandering around the house. During the appointment, they guide children through several realistic learning activities, including teaching speech, practicing with animal flash cards, using letter and counting flash cards, reading toddler picture books, and encouraging conversation practice.
Each appointment lasts around an in-game hour and feels like a genuine therapy visit rather than a standard social interaction.

Every Session Helps Your Sims Grow
The Speech Therapy Mod also gives every appointment lasting value. Toddlers improve their Communication skill while children develop their Social skill as they continue attending sessions.
Progress is saved for each individual child, meaning their therapy journey continues even if you save your game or age them up. After every appointment, players receive progress reports showing exactly how many sessions have been completed, such as “Session 3 of 6,” making it easy to follow each child’s development.
Halfway through the program, families receive a special milestone celebration, giving players another rewarding moment before reaching graduation.

Good Days and Tough Days Make the Journey Feel Real
One of the most touching features is that not every therapy appointment ends the same way. Most sessions finish successfully, rewarding children with the cheerful Found My Words! moodlet while parents and grandparents receive a Proud Caregiver moodlet celebrating their child’s progress.
However, the mod also recognizes that progress is not always linear. Some visits become Tough Speech Days, where children struggle to communicate despite everyone’s best efforts. During these moments, caregivers receive a Worried About Their Voice moodlet, and the therapist provides an honest report about the session.
These different outcomes make each visit feel more authentic while adding emotional storytelling opportunities for family-focused saves.
Homework Makes Families Part of the Process
Therapy does not stop once the speech pathologist leaves.
After every appointment, the therapist assigns speech homework that caregivers can complete by selecting Practice Speech Sounds (Homework) while interacting with the child. These practice sessions become sweet family moments where parents help their children continue learning between appointments.
Completing homework not only builds skills but also increases the likelihood that future therapy sessions will have positive outcomes, rewarding players who stay involved in their child’s progress.
Graduation Day Celebrates Every Achievement
The entire speech therapy journey builds toward a meaningful graduation after six completed sessions.
When graduation day arrives, the speech pathologist presents the child with a graduation certificate to celebrate all of their hard work. Children also receive the Found Their Voice moodlet, while the rest of the family joins in with a heartwarming They Did It! celebration.
Because progress carries across saves and even through aging, graduation feels like a genuine achievement that reflects the time players invested in their Sims.
Optional Speech Delay Feature Adds More Storytelling
Players who enjoy more realistic storytelling can also enable an optional Speech Delay system.
By selecting Note a Speech Delay on a child, Sims may occasionally experience a gentle Hard to Find the Words moodlet during everyday life. As therapy sessions continue, these moments become less frequent, showing gradual improvement over time.
Completing the full therapy program permanently removes the speech delay, although players can also remove the note manually whenever they choose. Since this feature is completely optional, it can easily fit different storytelling styles.
Built for Family Gameplay
Another thoughtful touch is that the entire household shares in the experience. Parents and grandparents react to both successful sessions and difficult days through dedicated moodlets, making the child’s progress feel important to the entire family instead of only the Sim attending therapy.
The mod is fully compatible with the base game. Players who own the Parenthood Game Pack will also see additional flash card activities included during therapy sessions, while those without the pack will still receive fully functional appointments using the remaining activities.
The therapy service costs §75 per visit, similar to hiring other household services in The Sims 4.
Helpful Console Commands Are Included
For players who enjoy testing or storytelling, the mod also includes several useful cheat commands.
The speech.check command verifies that both the script and package files are installed correctly. speech.calltherapist books a therapy appointment through the console for the active toddler or child. Players can also view progress using speech.sessions, or manually adjust completed sessions for storytelling and testing purposes by setting a specific session count.
Final Thoughts
The Speech Therapy Mod is one of those family gameplay additions that focuses on meaningful storytelling rather than simply adding another interaction. From realistic therapy appointments and homework to emotional milestones and a heartfelt graduation ceremony, it gives toddlers and children a memorable journey that many family players will appreciate.
The mod is base game compatible, with optional flash card activities available for players who own the Parenthood Game Pack.
