The Functional Osimpic Mod by Kemzima introduces a brand-new healthcare gameplay experience focused on medically supervised weight management. Inspired by community requests, this standalone mod adds consultations, prescriptions, realistic side effects, refill appointments, and even long-term consequences that make the experience feel much more immersive.
Rather than simply changing a Sim’s weight, this mod builds an entire treatment journey that gives every decision real consequences, making it a great addition for players who enjoy realistic storytelling and healthcare gameplay.
Functional Osimpic Mod Starts With a Doctor Consultation
Before your Sims can begin treatment, they’ll need to schedule a Weight Management Consultation with a doctor. During the appointment, Sims can either be honest about their health or stretch the truth if they want to try getting approved for a prescription they may not actually need.
If approved, the doctor recommends a dosage based on your Sim’s weight, lifestyle, and overall health. Sims who aren’t overweight can still receive a prescription under certain circumstances. Celebrity Sims may use their fame to impress the doctor, while highly charismatic Sims can sometimes persuade their way into getting approved.
Once approved, Sims can purchase their prescription through the computer for 750 Simoleons. Every Osimpic pen contains three doses, providing treatment for three weeks.

Three Dosage Levels Change How Treatment Works
The Functional Osimpic Mod includes three different dosage strengths, each affecting gameplay differently.
Players can choose between a Low Dose (0.25 mg), Medium Dose (1 mg), or High Dose (2 mg). Lower doses have gentler effects on hunger, while stronger doses reduce appetite more aggressively but also increase the likelihood of unpleasant side effects.
Each injection remains active for seven in-game days and immediately begins influencing your Sim’s appetite after use.

Side Effects Make Every Decision Matter
Just like real medication, Osimpic doesn’t come without risks. Sims may experience nausea, fatigue, headaches, dizziness, frequent burping, and mood swings throughout treatment.
Higher doses increase the intensity of these symptoms, and Sims who continue overeating while taking the medication may experience an even rougher recovery. These gameplay mechanics add another layer of realism for players who enjoy consequence-driven storytelling.
Long-Term Use Can Lead to Osimpic Face
One of the most unique features in the mod is the addition of “Osimpic Face.”
Sims who remain on the medication for too long, or continue taking it without truly needing it, may eventually develop noticeable facial changes through reduced cheek fullness. To reverse these effects, Sims must book a Facial Rejuvenation Treatment that costs 5,000 Simoleons. Kemzima has also shared plans to make this facial change optional in a future update.

Refill Appointments Keep Treatment Going
When an Osimpic pen runs out, Sims won’t simply buy another one. Instead, they’ll return to the doctor for a refill appointment.
During the visit, Sims can continue their current dosage, request a stronger prescription, or lower their dosage. If a Sim has lost too much weight, the doctor may recommend ending treatment altogether. Players can accept the medical advice or dispute the recommendation if their Sim wants to continue.
