Ever wondered whether your Sim actually needs those glasses you gave them in CAS? The Check Eyesight mod by simpishly gives you a simple way to find out, adding a little extra storytelling to your Sims 4 households.

Instead of turning eyesight into a full medical system, this mod keeps things nice and simple. Your Sims can discover whether they have good or poor eyesight, and Elders can even experience changes in their vision as they age.

Check Eyesight Gives Your Sims a Chance to Need Glasses

The Check Eyesight interaction is available for Sims from Infants through Adults. Just click your currently selected Sim, open the Actions menu, and choose Check Eyesight to immediately generate a notification and one of two hidden traits.

By default, Sims have a 70% chance of getting Good Eyesight and a 30% chance of getting Poor Eyesight. Sims with Good Eyesight simply carry on as normal, while Sims with Poor Eyesight are encouraged to wear glasses in CAS.

There’s also a little extra storytelling attached to the Poor Eyesight trait. These Sims receive a temporary +2 Fine buff and unlock a special Small Talk interaction called “Enthuse about Wearing Glasses.” It’s a small detail, but it can make a difference when you’re building out a Sim’s personality and story.

Elders Can Experience Changes in Their Eyesight

The mod gets a little more interesting once your Sims reach the Elder life stage. Elders can use the “(Re)Check Eyesight” interaction, which uses a separate system because their eyesight may have changed with age.

There’s a 30% default chance of receiving the Unchanged Eyesight trait and a 70% chance of receiving Old Eyesight. If their eyesight remains unchanged, nothing else happens beyond the notification. But if they receive Old Eyesight, you’ll be encouraged to add reading glasses to their outfits in CAS.

Elders with Old Eyesight also receive a temporary +2 Fine buff and unlock a special Complaint interaction called “Complain about Old Eyes.” Honestly, getting older in The Sims just keeps getting more relatable.

A Simple Storytelling Mod Without a Full Eye Care System

One of the reasons simpishly created this mod was to keep things much simpler than a full eyesight or medical gameplay system. The creator specifically explains that while adeepindigo’s Eye Care mod is great, they wanted something focused on one simple question: does this Sim need glasses or not?

That makes Check Eyesight a nice option for Simmers who want the storytelling detail without adding optometrist visits or eye diseases to their gameplay. It also won’t negatively affect Sims who technically need glasses but don’t wear them, so you can treat the system as a storytelling tool rather than a strict gameplay requirement.

Customize the Eyesight Chances

If you want more control over your Sims’ stories, the creator has also included a way to customize the chances. Using Sims 4 Studio, you can find the two Action Tuning files named “loot_Trait_Rando” and change the base chance number out of 100.

This means you can adjust how often Sims receive the Poor Eyesight or Old Eyesight traits. You can also use cheats or a mod such as MCCC to add or remove the hidden traits from Sims just like other traits.

Final Thoughts

Check Eyesight is one of those small storytelling mods that can add a surprisingly nice detail to everyday Sims gameplay. Whether you’re creating a child who discovers they need glasses, an adult who has always had poor eyesight, or an Elder whose vision is changing with age, it gives you another little piece of personality to play with.

The Check Eyesight requires the XML Injector, so make sure you have it installed before adding the mod to your Mods folder.
